GRANT REQUEST DEADLINES & SEMI-ANNUAL REVIEW MEETING DATES

ALL GRANT REQUESTS MUST BE RECEIVED BY:

MARCH 1, 2013 FOR OUR APRIL 2013 REVIEW MEETING

SEPTEMBER 1, 2013 FOR OUR OCTOBER 2013 REVIEW MEETING

Proposal Format
Proposals should be submitted in the following format:

1. Completed Grant Application Form
    a. Proposal statement
    b. A copy of your official IRS letter with your tax determination

Note: An audited financial report and current year operating budget may be required

Proposal Statements
Proposal Statements (second item in the above format) should answer these questions:

  • What are the objectives and expected outcomes of this program/project/request? 
  • What strategies will be used to accomplish your objective?
  • What is the timeline for completion? 
  • If this is part of an on-going program, how long has it been in operation?
  • What criteria would you use to determine your success?
  • If the request is not fully funded, what other sources can you engage?

A proposal budget should be included if this is for a specific program within your annual budget.  Please describe any collaborative ventures.

Communication
Requestors will receive a letter acknowledging our receipt of their request.  When a grant is approved grantees must sign a Grant Agreement Form stating that the funds will be used for the purpose intended, and return the form to us prior to our distribution of funds.  Progress reports and completion reports must also be filed.  All current grants must be in good standing, including status reports, prior to submitting new proposals.
